Project Manager
Jake Tanumihardjo (Tah-new-me-hard-jo), MPH, is the project manager for a Merck Foundation initiative called Bridging the Gap: Reducing Disparities in Diabetes Care. He works in the National Program Office at the University of Chicago. Jake earned a BS in biology and global health from the University of Wisconsin-Madison and an MPH in public health administration and policy with a focus on analysis and global health from the University of Minnesota. He has held research positions at the University of Wisconsin, Allina Health, and the Minneapolis Health Department in addition to serving as a project director for an Iowa State Innovation Model (SIM) grant at Great River Health. He has focused on a wide variety of topics including social determinants of health assessment and integration, community health needs assessments, public health programming and implementation, Vitamin-A bioavailability and bioefficacy, underage drinking, chronically ill frail elderly categorization, care coordination, and improving access to healthy foods.
